Essentials on the day

To avoid the hassle of refiling forms or re-collecting samples, please ensure that you have prepared all the items in this checklist.

  1. Examination fee (Less cash payments as possible. We accept credit card payments and ATM transferal. Mobile payments is not acceptable)
  2. Questionnaire for Health survey (must be completed before the examination)
  3. Stool sample
  4. Identification (national ID card, national health insurance card or any identification documents. Related identification for certain preferential treatments if eligible)
  5. Consent forms (including endoscopy consent forms. Please sign the anesthetic colonoscopy consent form, if the anesthetic procedure is intended)
  6. Driving to examination yourself is not recommended. Please take public transport or pick-up by the family (parking in the New Building B1 parking lot only open once).
  7. Examinees with any chronic illness (hypertension, cardiovascular disease, diabetes, asthma, etc.) in need of long-term medication should hold the usage once in the morning of the day of examination (asthma inhalers can still be used). Bring the medications with you and take them 60 to 120 minutes after endoscopy examination.
  8. Remove all nail polish.
  9. Examination gowns will be provided by the HMC.
  10. Do not wear contact lenses due to ophthalmologic examination.
  11. Bring a pair of sunglasses or an umbrella to alleviate the discomfort of photophobia after pupil dilation.
  12. Do not bring valuables.
